Reablement Support Officer, Part time

Learn more - here Our Reablement Support Officers work with a number of people in the community each day. You’ll travel to the people’s homes to support them with regaining skills and confidence to enable them to become more independent, actively promoting their choice and dignity. It’s an incredibly rewarding role, every day and every visit are different You’ll work with autonomy and independence but within a close-knit team, and you’ll have regular support from the Reablement Team Manager. You’ll also work closely with our health partners such as the intermediate care team. Shift Pattern Cycle: 4 days on, followed by 4 days off Shift Breakdown (per 4-day block): 2 Morning Shifts, 2 Evening Shifts Total Weekly Hours: 16/31.5 hours Shift Times: 16hrs Morning Shifts: 07:00 – 11:30 Evening Shifts: 18:00 – 22:30 Shift Times: 31.5hrs Morning Shifts: 07:00 – 16.00 Evening Shifts: 13:30 – 22:30 At £14.88 -£15.68 per hour, depending on experience. Your office base will be Chedworth Close in Ecton Brook, Northampton. We pay for whole shifts (not just individual visits) and pay up to 45p per mile. Our Benefits Generous annual leave entitlement starting at 28 days annual leave per year, 29 days for senior managers (pro rata, depending on how many hours per week you work), plus bank holidays. An additional 5 days, linked to continuous service, and ability to purchase up to 6 weeks additional leave per year. Local Government Pension Scheme plus immediate life cover (3 times your annual salary from day one) and ill-health protection. We pay for whole shifts (not just individual visits) and pay at 45p per mile. Cycle to work scheme. What will you be doing? Day to day, you’ll see multiple individuals in their own homes; you’ll sometimes visit with a colleague but mostly you’ll work on your own. You’ll then visit them regularly to monitor progress and support them with day-to-day activities which could include personal care, meal preparation, mobility and accessing the community with the aim of enabling them to regain their independence. You’ll case manage Reablement journeys, from initial assessment through to discharge. You’ll meet people after referral to assess their needs and support each individual to and support in the planning of their Reablement goals. You’ll also support people by signposting them to other agencies where appropriate, for example Assistive Technology, Age UK, Northamptonshire Carers etc. About you You’ll be confident working independently and have the ability to handle unexpected situations or challenges, being able to identify the source of a problem and find an effective solution. You’ll need previous experience within a health or social care setting, and either currently have, or be willing to work towards, NVQ level 2 in Health & Social Care or equivalent. A full driving license and access to your own vehicle is required, as you will travel countywide within Northamptonshire. We will offer a comprehensive induction to the successful candidate. This role is exempt from the Rehabilitation of Offenders Act 1974 and therefore subject to an enhanced DBS certificate, at our cost.
