Job TitleBand 6 District Nurse Caseload Holder (Secondment for 12 months) Job OverviewThis post is a 12‑month secondment to meet the needs of the service. If you are interested, you must obtain permission from your current line manager prior to applying. The role is a Band 6 District Nurse Caseload Holder working within the Deeside District Nursing Team. The post holder must hold or be in the second year of a Community Specialist Practitioner qualification and be a Registered Nurse Practitioner. The role involves working jointly with Nurse Case Managers who are based within the District Nursing team. Ability to speak Welsh is desirable; Welsh and/or English speakers are equally welcome to apply. Responsibilities Manage a defined caseload in line with good practice and take responsibility for a 24‑hour period. Conduct complex needs assessments (e.g. CHC, health and social care) and provide specialist district nursing practice. Collaborate with other agencies to provide patient services without direct supervision. Undertake a range of management duties and deputise in the absence of the Team Leader. Hold a Specialist Practice Qualification in District Nursing or be working towards it, and be a registered Nurse Prescriber. Qualifications Essential criteria NMC Registered first level nurse District Nursing / Certificate, diploma, degree or working towards Nurse prescribing qualification or working towards Evidence of ongoing education and professional development Mentorship and assessment training Desirable criteria Management qualification e.g. ILM, RCN Leadership Experience Essential criteria Community experience as Band 5 or equivalent Experience working with patients who have acute or complex needs Varied clinical experience and multidisciplinary working Desirable criteria Link nurse role Aptitude and Abilities Essential criteria Analytical skills and ability to interpret clinical signs Organisational skills Ability to assess, plan, implement, evaluate and delegate appropriately the delivery of care Supervise, teach and assess registered and non‑registered staff Provide mentorship and preceptorship as appropriate Ability to work collaboratively with members of the primary health care team and other agencies IT skills Awareness of the changing health agenda, research and its application to practice Knowledge and evidence of application of consent Knowledge of data protection and patient confidentiality Understanding of risk assessment and health and safety issues Desirable criteria Experience of undertaking audit Recognised qualification within an area of chronic conditions management Ability to speak Welsh Awareness of the wider health agenda in which care is provided Values Demonstrate ability to care and communicate with warmth and understanding Excellent verbal and written communication skills Committed to continuous development Other Flexible approach to the needs of the service Demonstrate timely travel arrangements between patient’s homes, clinics and surgeries #J-18808-Ljbffr
