The service supports individuals to thrive in their own environment - whether that be via preventing admissions and increasing skills to support independence or offering support and rehabilitation following hospital admission. This role is key to the success of the team and will work alongside Physiotherapists, Occupational Therapists and support workers to deliver the highest quality goal‑focused rehabilitation. The service is a 7 day service with operational hours between 8am and 6pm. Weekends and later finishing shifts are scheduled via a rota. The role of the assistant practitioner is patient facing, delivering assessments once competencies have been completed. Goal focused treatment plans are created and progressed. Each AP caseload manages as small non complex caseload - will support from a qualified clinician. While the ‘home team’ will be based in Rushcliffe - there may be need to work outside of this locality - as determined by patient need., The role is primarily concerned with creating and delivering goal focused treatment plans. Our client group is very varied and while it is mainly older adults - the service supports those over 18 years old - with functional difficulties. The role is diverse and a competency framework in in place to support skill acquisition to enable the delivery of this care. Care delivery also occurs in a clinic environment and maintaining accurate records and supporting the wider team to safely manage the waiting lists is a key part of this role., Responsibilities Deliver patient‑facing assessments once competencies have been completed. Create and progress goal‑focused treatment plans. Support patients and carers on the waiting list for the service. Maintain accurate records and know your limitations and when to escalate risk and safeguarding concerns to the registered team members. Act professionally at all times and demonstrate a positive image of the service. Support the team to patients and carers on the waiting list for the service. Be confident speaking to members of the public and be able to modify your communication style as required to the situation. Be required to have some knowledge and experience of working with people older adults and be able to offer support and care to them whilst they are treated by the service.
