Excellent communication skills are essential to this role.Main duties of the jobKnowledge of needs of patients with long-term conditionsCervical cytology, immunisation and vaccination skillsSpirometry monitoringChange management skills and ability to support patients to change lifestyleExcellent communication skills, both written and verbalAbility to communicate difficult messages to patients and familiesAware of accountability of own role and other roles in a nurse led serviceKnowledge of health promotion strategiesAwareness of clinical governance issues in primary careKnowledge of patient group directionExperience of TPP S1Membership of a professional bodyMotivated and enthusiasticWilling to learn new skillsFlexibleAdaptable to and embracing of changeExperience of working in a Primary Care environment is preferred.About usKingston Health is a friendly practice of 10500 patients based over 2 sites covering both west and east of the city.We have 4 GP partners, 1 Physician Associate. 1 Pharmacist , 2 Practice Nurses, 2 Health Care Assistants supported by ourknowledgeable administrative team. We are also a training Practice with F2s.We offer the NHS pension scheme.Job responsibilitiesDuties andResponsibilitiesClinicalTomanage patients with chronic disease, running specific clinic sessions andmaintaining robust call and recall systemsParticipatein national screening work such as cervical cytology and deliver the nationalimmunisation and vaccination scheduleProvidetravel health advice and administer appropriate vaccinationsProvidedirectly and assist GPs with contraceptive servicesInteractwith patients/parents/carers and provide appropriate ongoing support to meetidentified needsUndertakehealth promotion to help educate patientsSelect,carry out and interpret results of specific tests if requiredTomaintain accurate patient records and enter onto the computer using Ardens templatesEnsureaccurate completion of all necessary documentation associated with patienthealth careTocollect data and participate in regular Practice audit and research projectsTocontribute to the preparation of any Practice development plansTomaintain own annual registration with the NMC and adhere to Code ofProfessional ConductTomaintain a safe and therapeutic environment for all nursing team staff,patients, carers and co-workers from other agencies.
