Diabetes Specialist Nurse, Band 6 – North Cumbria Integrated Care NHS Foundation Trust Job Summary The post holder will be an integral member of the Diabetes Service specialist team, providing care, treatment and education to diabetes patients and their families. This will involve working closely with the community network of health care professionals to deliver an integrated package of care between hospital and primary care. The Diabetes Service represents an integration of the entire multidisciplinary specialist team with responsibility for the quality of all diabetes services in Cumbria. We aim to improve the quality of diabetes care by delivering patient pathways with structured education programmes to support self‑management, by supporting and training of primary care teams and by providing excellent specialist diabetes clinical services in both in‑ and out‑patient settings. The post holder will be based in Carlisle but will be required at times to travel across North Cumbria. Responsibilities A key aim of the Diabetes Service is to facilitate and encourage self‑management by people with diabetes. Structured patient education programmes (DAFNE and DESMOND) are embedded in our pathways of care and training has been delivered to both primary care and specialist staff to undertake holistic care planning. We provide speciality services including pregnancy, young person/transition service and an insulin pump service. A Cumbria‑wide retinal screening service operates throughout Cumbria at a variety of locations. Person Specification Essential Qualifications Degree level education in Nursing NMC registration Willing to work towards diabetes‑specific qualification (e.g. Warwick Course A205, CIDC, Bradford Diabetes course) Willing to undertake educator training in DESMOND DAFNE Desirable Qualifications NST307 or equivalent Diabetes‑related qualification (Warwick Course A205, CIDC, Bradford Diabetes course or equivalent) V300 prescribing Essential Knowledge and Experience Significant general nursing experience within the NHS Detailed knowledge of all aspects of diabetes and associated risk factors Ability to work autonomously, efficiently and professionally locality‑wide visiting patients in General Practice surgeries, Health centres, community and acute hospitals and emergency home visits Experience of teaching adults, general IT skills including experience of Windows‑based software and MS Office Understanding of current NHS issues and initiatives Knowledge of clinical governance and its application Up‑to‑date knowledge of developments in diabetes care, including hybrid closed‑loop insulin pumps Knowledge of risk factors associated with diabetes Desirable Knowledge and Experience Willing to become a DESMOND/DAFNE/Walking Away trainer Experience of undertaking clinical audit Experience implementing diabetes‑specific treatment regimens, e.g. insulin infusion Inpatient nursing experience Essential Skills and Aptitude Demonstrable clinical skills within specialist area, including physical assessment skills and basic life support Ability to obtain and evaluate information to aid decision‑making Effective organisational skills to work across different areas of the service Ability to work in isolation but as part of a team Co‑operative, flexible and diplomatic Ability to manage difficult or emotional situations Desirable Skills Familiarity with and implementation of diabetes‑specific treatment regimes, e.g. insulin infusion Other Requirements Able to travel independently of public transport and be able to travel widely as and when required Willing to undertake training as required Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
