We are looking to recruit Care Coordinators who willwork as part of the multi-disciplinary team in the Barnsley Primary Care Network,building on our successful and existing model.Role will be to work closely withPCN Practices and the multidisciplinary team in coordinating all key activitiesincluding access to services, advice, and information, and ensuring heath andcare planning is timely, efficient, and patient-centred.This is a full time role of 37.5 hours at Royston High Street practice S71 4RF.If a high number of applications are received this advert may be closed early.Main duties of the jobThe post holder will help patients interact and engage witheveryday life through activities designed to develop, maintain, or retrainskills for people with a cognitive, physical, or mental disorder, condition, orillness. You will support the provision of continuity of care and act as apoint of contact for families, residents, and professionals for identifiedpatients in your caseload.The ideal candidate will have experience working in PrimaryCare and possess excellent communication and organisation skills, with ademonstrable passion for enhancing patient experience and outcomes.The ability to travel to local care homes as part of the role is essential.About usBarnsley Primary Care Network:Barnsley has established onesuper Primary Care Network (PCN) of over 250,000 patients which includes allof GP practices across Barnsley. If a suitablecandidate is appointed the role may close early, therefore please do nothesitate to submit your application.Job responsibilitiesKeyresponsibilitiesWork with people,their families and carers, to improve their understanding of their condition.Support people to develop and review personalised care andsupport plans to manage their needs and achieve better healthcare outcomes andcontinuity of care.Help people tomanage their needs by providing a contact to answer queries, make and manageappointments, and ensure that people have good quality written or verbalinformation to help them make choices about their care.Signpost patientswhere appropriate to other additional roles within the PCN for example SocialPrescribers and Health & Wellbeing coaches.Provideco-ordination and navigation for people and their carers across health and careservices. Helping to ensure patients receive a joined-up service and theappropriate support from the right person at the right time.Workcollaboratively with GPs and other primary care professionals within the PCN toproactively identify and manage a caseload, which may include patients withlong-term health conditions, and where appropriate, refer back to other healthprofessionals within the PCN.Support theco-ordination and delivery of multidisciplinary teams with the PCN.Raise awareness ofhow to identify patients who may benefit from shared decision making andsupport PCN staff and people to be more prepared to have shared decision-makingconversations.Process and bookappointments by telephone or written format as requested via practice.Process referralsto community nursing and other professionals.Person SpecificationExperienceGood understanding of GDPR and confidentiality policies.Knowledge and understanding of Primary Care and Primary Care Network.Experience working with healthcare professionals and/or previous experience in a GP practice or in the primary care sector. `Experience coordinating with multiple stakeholder or individuals to meet specified outcomes.Experience providing advice/signposting.Experience using a patient clinical system.Knowledge of a range of local community groups which support wellbeing.Awareness of relevant Health and Social Care legislation and a developed knowledge of crisis intervention.SkillsExcellent written and spoken skills with the ability to communicate effectively at all levels including with patients, carers, specialist services, GPs and colleagues.Good technical literacy with Microsoft Office packages.Able to work independently and manage own workload.Able to build strong professional relationships.Demonstrable experience of effective planning and organisation skills to deliver targets and meet deadlines.Able to analyse and interpret information and present results in a clear and concise manner.Understanding of social determinants of health and how these can be addressed with patients.QualificationsEducated to GCSE level or equivalentDeposition/Approach to workCreative, flexible and sensitive approach to working with people with diverse support needsAbility to motivate peopleAbility to reflect on and share best practice with peersAble to travel locally as required
