Job summary We have an exciting new opportunity for a D igital and Transformation Lead within Bromley by Bow Health & Stepney Health CIC and Healthy Island Partnership, Tower Hamlets. This new role will lead transformation work across two PCNs and use Digital where appropriate to enable the change and maximise the use of the tools available. The Digital and Transformation Lead will work with clinical and operational teams within two Primary Care Networks (PCNs) to improve patient access, support the integration of technological solutions to help practice teams manage workload, improve staff and patient satisfaction, and the efficiency and sustainability of the PCN and Practice services. You will be working on local and national priority areas in a fast paced moving environment. Interviews will take place the week commencing 12 January 2026. What Youll Do Build relationships across the PCNs (clinical and non-clinical) to develop consensus for improvements and transformation Improve adoption and/or optimisation of new and existing digital technologies to enhance patient access and experience Identify opportunities to improve the efficiency of clinical and non-clinical process Identify opportunities for continuous improvement to support practices and PCNs manage workload Support and improve the digital maturity of both PCNs Use data and insight to improve population health management Responsibilities Lead opportunities for continuous improvement to support practices and PCNs manage their workloads Build relationships across the PCNs (clinical and non-clinical) to develop consensus for improvements and transformation Facilitate collaboration between practices, PCNs and the wider system to support at-scale or appropriate local-level, delivery of care to patients Attend meetings of PCN/s, Practices and the strategic health system, when required/appropriate Lead on CAIP development and monitoring across both PCNs Support team members that are involved in the procurement of new technologies for the PCNs/Practices Crunch and decipher data to support programmes and transformation initiatives within PCNs/Practices Report regularly to PCN teams on progress and developments Support staff to identify and participate in relevant training and/or professional development opportunities that will support transformation initiatives Support other ARRS Teams within the PCNs to embed digital developments with-in their working processes What Were Looking For We are looking for a dynamic and innovation individual with experience in: Data collection and analysis Quality Improvement Primary Care Project Management Why Join Us? Experience Previous experience in capturing and analysing data to improve programmes/projects in a healthcare setting. Experience of working on projects, successfully delivering change and improvement programmes. Knowledge and Skills Working and/or technical knowledge of IT products such as primary clinical systems and digital tools. Qualifications Project management qualifications or relevant experience.
