We have a rewarding opportunity available for Wellbeing Coordinator to join the team within our Cambridgeshire Community Navigators Service. Our Community Navigator Team offer person centred support to clients to help them live well and achieve their individual outcomes. The team use their local knowledge to link people in with services and use their knowledge and expertise to provide advice, information, guidance and signposting. Responsibilities As the Wellbeing Coordinator, your role will focus on building coping strategies and resilience, as well as widening networks of support for People We Support. You will provide guidance and information for individual needs using a range of methods including face-to-face appointments, telephone support, virtual appointments and via email/text. If you're ready to join our team and help make a real difference, then apply now, and let's get started on this journey together Qualifications Candidates must have an NVQ level 2 (or equivalent) in Health and Social Care or willingness to work towards this. Previous experience of working with individuals to assess their needs and produce plans to meet outcomes is essential for this role.
