Job Summary We have an exciting opportunity for a forward‑thinking Band 6 senior Occupational Therapist to join our Integrated Therapy Team in an acute and community setting. You will manage a diverse, complex caseload within a dynamic environment, providing a high standard of occupational service in designated acute inpatient, outpatient and community settings as part of an annual rotational scheme. Main Duties Support rotational OTs. Complete specialist assessment, rehabilitation and discharge planning. Plan and organise a designated caseload of complex patients within the specified service, owning professional activities under the Team Leader. Work with the patient and carers to identify and meet OT goals, using standardised tools to assess skills deficits and achieve functional fitness for timely discharge. Plan and implement single or a programme of interventions to meet patient needs, monitoring, evaluating and modifying treatment to ensure effectiveness. Ensure Therapy goals are integral to the multidisciplinary care plan for safe and timely discharge. Provide advice and education regarding lifestyle changes and adaptations for patients’ physical and social environment.
