Haven Health and Howard House surgery in Felixstowe have multi-disciplinary teams who are serious about health care and achieving the best for all their patients but who like to work in a supportive and fun atmosphere. We are seeking to recruit an enthusiastic and flexible Practice Paramedic to work in the community treating both Haven Health and Howard House patients.Suitable candidates must be able to work flexibly, autonomously and proactively in the community supporting both our housebound and care home patientsThe vacancy is for approx. 15 hrs a week; however, flexibility is crucial to accommodate training, holidays and occasional sickness.Within easy access of Ipswich, (20 minutes). We have established teams with a strong focus on customer service and communication. We would love to hear from candidates with GP surgery experience.Main duties of the jobThe main duties will be treating both housebound and care home patients across 2 surgeries. Leading on the various vaccination campaigns. Completing referrals where applicable. Working alongside non-clinical team membersTo be responsible for seeing patients across multiple practice sites, care homes and for visiting patients in their place of residence, who present with a wide range of minor illness, urgent and same day requests for treatment and chronic disease management; providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. There will be the need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.KeyResponsibilitiesEnsure all medical notes are written contemporaneously and succinctlyInduct and arrange training for existing team members and new employees where applicable.Arrange flu and immunisations for all care home and housebound patients.Provide assessment, diagnosis and treatment at first point of contact by attending to patients according to their needs in the patients homes/Care homes. Act as an autonomous clinician, with the support and guidance from the GPsProactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition (as appropriate).Diagnose and manage both acute and chronic conditions, integrating both drug and non‑drug based treatment methods into a management plan.Accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.Recognise, assess and refer patients presenting with mental health needsTelephone triage as appropriate for clinics and home visits and care home ward rounds and provide access to GPs as necessary.Evaluate clinical information from examination and history taking and initiate appropriate treatment and / or referral, including admission to hospital if required.Signpost appropriately to other services in the identified healthcare pathway and to refer patients to appropriate care settings, within locally agreed healthcare pathways.Maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.Advise and support members of the public on healthy lifestyles, health promotion and disease prevention.Provide clinical advice to other members of the team where appropriate.Work in the clinic setting using AskmyGP as requiredAny other duties within the scope of the post holder as requested by the management team.Person SpecificationQualificationsRegistered ParamedicSystmone TrainedExperienceIdeally with previous experience of working in primary care.
