Overview An exciting opportunity has arisen to join the District Nursing Service for a Community Staff Nurse covering the Out of Hours service in North Tyneside and Northumberland. We are looking for enthusiastic staff nurses to join our district nursing teams based at The Health and Care Academy. Responsibilities Work autonomously and as part of a team, reporting directly to the line manager and the community nursing team lead. Assess, plan, implement and evaluate care for a diverse group of patients with a wide range of conditions and health needs. Develop and deliver programmes of health and social care. Provide care and support to patients with a terminal illness and their families, including emotional and bereavement support. Perform skilled nursing care including assessment, wound management, palliative care and catheterisation. Conduct diagnostic tests such as Ankle Brachial Pressure Index using Doppler and interpret results. Perform procedures requiring fine dexterity: venepuncture, drug administration by injection, removal of sutures. Interpret results (e.g., blood glucose estimation) and communicate them to patients. Explain aspects of care to patients and carers, allowing questions and concerns. Maintain high standards of professional practice and share approaches and experience with others. Maintain a duty of care to patients, carers and colleagues with personal and professional accountability. Qualifications State Registration on Part 1 of the 3 part NMC register. Additional clinical qualifications or equivalent derived through experience at diploma/degree level. Newly qualified staff must undertake the Trust preceptorship programme within the first year in post. Evidence of qualification to undertake supervision, teaching and assessment of students (essential; expected completion within 18 months). Other Requirements Valid driving licence and ability to use a vehicle or Trust personal lease vehicle for work. Trust may consider reasonable adjustments to enable a disabled person to undertake the role. Skills and Abilities Excellent written and verbal communication skills. Strong interpersonal skills. Organisational skills to manage own work and the work of others. Demonstrated English language proficiency level required for the post. IT skills to enter information in clinical systems. Experience & Knowledge Experience working independently in a healthcare environment. Ensuring the delivery of “Compassion in Practice” nursing and care staff vision and strategy, incorporating the 6C’s. Knowledge or experience of coaching and mentoring practices and tools. Knowledge or experience of quality improvement tools, techniques and methods. Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a DBS check for any previous criminal convictions.
