Overview Join Our Older Persons Mental Health Team in Torbay. Our well-resourced, multidisciplinary team supports older adults with functional mental health conditions and organic brain disorders in a beautiful coastal location. You will work in a collaborative environment with experienced clinicians, focusing on delivering compassionate, person-centred care in the community. Responsibilities Coordinate holistic care for older adults with functional mental health conditions and organic brain disorders. Develop and maintain personalised care plans reflecting patients' needs, preferences, and goals. Serve as point of contact for patients, families, carers, and professionals across health and social care services. Link patients to appropriate services, including GPs, social workers, therapists, and voluntary sector support. Review care plans regularly and adapt them as conditions or circumstances change. Work closely with multidisciplinary teams (nurses, occupational therapists, psychiatrists, psychologists, social care professionals). Conduct mental health assessments, especially for patients with complex needs or cognitive impairments. Support patients in managing their conditions and promoting independence through education and engagement. Ensure continuity of care during transitions between services or after hospital discharge. Maintain accurate records and documentation; conduct home visits as needed to assess needs and provide support. Person Specification Essential Registered with the NMC or HCPC. Degree in Mental Health Nursing or Occupational Therapy. Experience in mental health settings, ideally with older adults. Knowledge of the Mental Health Act, Mental Capacity Act, and safeguarding procedures. Ability to conduct comprehensive assessments and develop care plans. Strong communication and teamwork skills. Proficient in using electronic patient records and digital tools. Desirable Experience in Community Mental Health and autonomous community work (home visits, liaising with primary care). Caseload management skills and ability to manage complex cases. Non-Medical Prescribing qualification (for nurses) or willingness to undertake training. Knowledge of Older Adult Mental Health Conditions and coordinated care across services. Crisis management and risk assessment capabilities. Experience working with multidisciplinary teams and delivering integrated care. Commitment to professional development and ongoing learning. Qualifications Essential Registered with NMC or HCPC.
