Job Summary This is an exciting opportunity for an experienced Advanced Physiotherapy Practitioner to develop their clinical practice in a leadership role. Team & Setting You will work directly with a small team of First Contact Practitioners to patients across our Primary Care network of Practices in South Bristol. You will need to have obtained or be in the process of obtaining an Advanced Practice qualification (post‑grad diploma or degree, or Masters if qualified after 2020). Job Details Date posted: 08 December 2025 Pay scheme: Other Salary: £50,952 to £57,349 a year depending on experience Contract: Permanent Working pattern: Full-time, Part-time, Flexible working Reference number: A1833-25-0026 Job location: Brooklea Health Centre, Wick Road, Bristol BS4 4HU Main Responsibilities Clinical Independently triage, assess, evaluate and diagnose patients with highly complex disorders and multiple pathologies using advanced critical thinking and diagnostic reasoning skills, including initiating appropriate investigations and interpreting results to develop immediate management plans that meet the physical and psychological needs of the patient and family. Justify clinical referrals to specialist services (acute and specialist services such as surgical orthopaedics, rheumatology, neurology, other AHP services, nursing services and the primary care medical teams). Recognise red flags, serious pathology and potential underlying non‑MSK related disease that may present as MSK symptoms and refer appropriately, accelerating patient referral when needed. May initiate medication prescribing, administration and management in line with Patient Group Directives or prescribing as required for a specialist role; NMP qualification desirable but not essential. Maintain timely and accurate patient record documents to reflect care provided and to be professionally and legally accountable. Work collaboratively with practice and PCN colleagues in a multidisciplinary team, maintain relationships with orthopaedic consultants, relevant MDT and physiotherapy services in community and acute settings, and develop relationships with wider health and social care agencies where appropriate. Actively adopt a personalised care and population‑centred care approach to enable shared decision making with the patient. Engage in care from a population care viewpoint. Service Development & Education Lead and develop the service to improve practice, implementing and evaluating evidence‑based standards, guidelines and policies. Identify and be involved in audit and research activity that contributes to the development of the service. Plan and lead specific areas of PCN/practice developments, including implementing best practice and management to enhance the patient journey. Promote a culture of continuous learning and improvement within the service and the team, identifying opportunities for service development. Facilitate education and training within the PCN. Provide supervision and support to members of the FCP team. Research & Audit Initiate and contribute to research in the specialist field that directly improves patient management and the service. Interpret newly generated research findings and implement them through advanced knowledge and skills, disseminating findings internally to influence best practice. Develop and undertake regular audits that promote and facilitate service improvement. Leadership Establish strong working relationships with physiotherapists and staff management to encourage good clinical practice. Work with GP leads of the five Practices to identify and follow up opportunities for improving care. Provide clinical support and advice to colleagues including care coordinators and physiotherapists. Person Specification Experience – Essential Clinical competency working with frail older people (including care home residents). Management of patients with long‑term conditions. Advanced clinical practice skills. Experience – Desirable Experience working in primary care. Nurse‑led management of minor illnesses and ailments. Developing protocols and clinical guidelines. Clinical supervision of staff. Qualifications – Essential Registered Nurse with the Nursing and Midwifery Council. Master’s degree required for qualification post‑December 2020. Post‑graduate diploma or degree for Advanced Practice Qualification up to December 2020. Qualified Independent Nurse Prescriber on the NMC register or working towards a prescribing qualification with completion expected within twelve months of appointment. Meet NMC revalidation requirements in accordance with the NMC Revalidation booklet. Meet the standards for registered ANP working at advanced level. Skills and Abilities – Essential Good IT skills, including use of GP clinical systems (EMIS Web). Comfortable working in a range of settings including GP surgeries, care homes and patients’ own homes with minimum support and supervision. Excellent communication skills to work with patients from a variety of backgrounds, healthcare professionals and care home staff.
