ACTsare teams of alcohol specialist clinicians, based in acute hospitals whichprovide specialist support, predominantly to alcohol-dependent patients. The post will require flexibility, working across Sunderland Royal andSouth Tyneside Hospital, you will also contribute to covering a 7 dayservice. The successful candidate will be able to demonstrate excellentcommunication skills, team working skills and will be dedicated to improvingthe patients' experience. You will need to demonstrate continuing professional development, some experience working with this patient group and ability to workclosely within a multi-disciplinary team. Working closelywith the Alcohol Care Team, the Alcohol Practitioner will assess the needs ofthe individual patient and deliver appropriate psychosocial interventions andrisk assessments. Provide training to employees onscreening and brief interventions and support the Alcohol Care Team in developingand delivering education to Trust employees in the speciality of alcohol misuse. Responsible for managing a caseloadof individual patients who are not engaging in the community but continue tocome to hospital to reduce alcohol health harms through a wide range of supportand interventions. Flexible to meet the needs of theteam and will demonstrate knowledge, expertise and passion in working withinthe speciality of alcohol misuse. To develop therapeutic relationships with clients andfamily/carers. To have knowledge of the impact excessivealcohol consumption has on physical health, mental health and social issues andto use this knowledge to fully assess patients. Toprovide brief interventions, psychosocial interventions and motivationalinterviewing as appropriate. To screen patients to identify those at riskfrom alcohol related health and social problems and those whose health is beingdamaged by alcohol. Improve data collection and opportunities for analysis. Todevelop strong professional relationships with colleagues, Trust employees,Trust volunteers and staff from external agencies. To support friendsand families who are struggling to cope with a loved ones alcohol misuse. To support other members in the Alcohol Team inthe care and management of intoxicated patients and patients in acute alcoholwithdrawal. To make accurate assessments of the risk patients poseto themselves and others and take necessary action within Trust safeguardingguidelines. To educate patientsand their families on alcohol related harms and deliver brief advice. Toprovide training to Trust staff on screening and brief interventions and toassist the Alcohol Team in developing and delivering education programmes. To makeappropriate referrals into community services. To adhere to theTrust equality, diversity and inclusion policy. To havethe ability to be flexible and to manage your workload effectively. Todemonstrate an ability to work under pressure. To elevate any concerns to the lead alcoholnurse in a timely manner. Todisplay excellent verbal and written communication skills and be computerliterate. To keeptimely and accurate records of interventions and outcome activity and use thisin decision making, using the Trusts policies, procedures and electronicrecording and data collection system. To signpost intoother services and integrate care with other providers of health services. Toundertake clinical supervision on a regular basis in line with relevantprofessional guidelines and policies. To complywith the Trusts vision, values and behavioural compact. Person Specification Experience Proven prior experience in working with clients with alcohol misuse issues. Experience of working within an acute hospital. Experience in providing training in brief interventions. Physical Skills Ability to assist patients to move as per Trust Moving and Handling Policy. Flexible to meet the needs of the service covering 7 days a week. Qualifications Maths and English (GCSE 9-4) or Maths and English Functional Skills Level 2. Level 3 NVQ in Health and Social Care or equivalent. Skills & Knowledge Knowledge and experience in delivering of psychological interventions including brief interventions. Knowledge of the impact alcohol misuse can have on physical health, mental health and social issues Ability to provide and receive complex, sensitive or contentious information. Ability to build constructive relationships with warmth, using good communication skills. Ability to be flexible and to manage own workload effectively. Ability to work under pressure. Ability to work within a multidisciplinary team. Ability to work in partnership with other agencies. Competent in the use of IT software such as Microsoft Office. Ability to deliver training and presentations as required to staff and service users. Knowledge of national health and social care policy. Ability to recognize when to elevate concerns to senior staff.
