Job Summary We're looking for a Mental Health and Wellbeing Practitioner (MHWP) to work within the Darlington Community Mental Health Teams (Community HUB and Community Treatment & Intervention Team). The MHWP will provide wellbeing-focused psychologically informed interventions, aligned to cognitive-behavioural principles for people with severe mental illness. Main Duties Work alongside service users, families, carers and multi-disciplinary team members to coordinate care and support collaborative decision‑making. Deliver wellbeing-focused, psychologically informed interventions, aligned to cognitive-behavioural principles, based on best evidence to address problems experienced by people with severe mental health problems. Work under close case‑management supervision from a suitably qualified professional to provide high‑quality collaborative care planning as a member of the multi-disciplinary team. Work under close clinical skill supervision from a suitably qualified member of the MDT. Knowledge and experience of working with people with mental health needs gained through a graduate or postgraduate certificate training with significant supervised practice. Able to plan and deliver interventions with a variety of service users presenting with mental health difficulties, their families and carers and the clinical team, on a daily basis. Experience working as part of a team. Experience of being supervised. Experience running groups/activities. Experience of analysing and communicating complex information verbally and in writing. Desirable Working with complexity in a mental‑health setting. Understanding of clinical risk management in practice. Experience around the role of the key worker.
