Practice Nurse with Chronic Disease ManagementWe are looking to recruit a Practice Nurse with CDM experience who is highly motivated, proactive and positive to join our clinical team. You will need to be competent in all the usual treatment room duties including wound care, compression bandage, cervical smears, baby and travel immunisations.We hold a PMS Contract in South GloucestershireWe use EMIS Web and ArdensWe provide a full range of enhanced services.We are high QoF achieversWe have an active working relationship with our PCN member practicesMain duties of the jobWe are looking for an experienced primary care nurse that is able to hit the ground running.It's a busy environment, so you will need to be able to work under your own initiative as well as part of a successful team. Additionally you will need to be flexible and amenable to the changing demands of primary care.About usSt Mary Street is a well established Practice located in the semi rural town of Thornbury near to the Severn Estuary. We are a dedicated team of 30 people (directly employed) who are supported by PCN employed colleagues including a Pharmacy team, Physiotherapists, a Frailty team and Social Prescribers.We are a friendly, successful 4 Partner Practice committed to providing high quality patient health and care services to our 9000 patients and growing. Our ethos is built upon some core values including safe, accessible and continuous care for each of our patients.We are also a GP training practice with 3 of our 4 Partners registered as trainers. Therefore, we commonly have learners with us at any given time. In addition, we are supported by three Salaried GPs, an Advanced Nurse Practitioner, a highly successful nursing team and a dedicated and supportive administrative team.We are a social bunch and fair well in the annual GP Patient Survey. Our staff turnover is low and the team supports and looks out for each other especially as patient demand on our services reaches an all time high.Job responsibilitiesImplement and embed health promotion and well‑being programmesIdentify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on patients healthProvide routine nursing care to patients as required in accordance with clinical‑based evidence, NICE and the NSFProvide wound care (ulcer/Doppler etc.) to patientsProvide immunisations (childhood, seasonal, etc)Provide cervical cytology screeningRequest and process pathology resultsMaintain accurate clinical records in conjunction with extant legislationEnsure accurate use of SNOMED codes to support QOF and LES/DES targetsChaperone patients where necessaryAssist GPs with minor surgery when requiredPrioritise health issues and intervene appropriatelySupport the team in dealing with clinical emergenciesImplement vaccination programmes for adults and childrenSupport patients in the use of their prescribed medicines or over‑the‑counter medicines (within own scope of practice)liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)Support the clinical team with all safeguarding matters, in accordance with local and national policiesUnderstand practice and local policies for substance abuse and addictive behaviour, referring patients appropriatelyDeliver opportunistic health promotion where appropriate.Person SpecificationExperienceExperience of working in a GP PracticeExperience of working within a nursing teamExperience of managing COPD, Asthma and/or DiabetesQualificationsRegistered NurseDiploma in COPD, Asthma and/or DiabetesPostgraduate Diploma or Degree (Primary Care)Disclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
