Perform strengths‑based assessments and develop effective care and support plans. Collaborate with multi‑disciplinary partners including health professionals, hospital teams, community services, and voluntary organisations. Support safe hospital discharges and prevent avoidable admissions. Undertake Mental Capacity Act assessments and safeguarding enquiries where required. Promote independence, resilience, and high‑quality outcomes for adults and their families. We are seeking a practitioner who is: A qualified Social Worker with SWE registration. Experienced within adult social care, ideally with knowledge of frailty, hospital discharge, or intermediate care pathways. Confident in risk assessments, safeguarding practices, and person‑centred planning. Skilled at working in fast‑paced, integrated environments.
