Join Our Chippenham Community Team Community Nurse Base: Chippenham Community Hospital The Chippenham Community Team is seeking a Community Nurse to join our experienced, multidisciplinary service providing care and support to people in their own homes and local community. This role involves delivering safe, effective, and high-quality nursing care to individuals with a range of complex needs. You will work closely with colleagues from nursing, therapy, and other health and social care disciplines to promote independence and improve health outcomes for our patients. What We’re Looking For Registered Nurse (NMC Registered). Frontline community care experience. Evidence of ongoing clinical and professional development. Strong clinical assessment, planning, and communication skills. Ability to manage workload effectively and maintain high standards of care under pressure. A flexible and professional approach to patient‑centred care. Additional Information You must hold a full UK driving licence and have access to a vehicle suitable for business use. Main Duties of the Job Participate in assessment and care planning of patient care in conjunction with the Team Manger and support of specialist team members. Implement and evaluate care and treatment in agreement with the patient and as defined in the care plan; escalates any deviation from the care plan or change in condition to the Team Manager / GP accordingly. Communicate effectively with patients, carers and all members of the multidisciplinary team. Prioritise daily work in line with clinical, patient need. Maintain accurate and legible patient records in line with professional and organisational requirements; this includes electronic patient record system and any paper records. Implement and work to ensure evidence‑based care, best practice. Actively participate and contribute to team meetings and daily handover. Initiate health promotion activities with individuals, patients and carers. Actively involved in the development and implementation of community nursing service developments and initiatives. Participate in clinical audit and the implementation of changes to ways of working / clinical practice as a result of findings, recommendations and action plans. Participate in risk assessment and promote the safety and well‑being of patients, staff and any other persons at all times and assist in ensuring a safe work environment.
