Senior Occupational Therapist/Physiotherapist Join to apply for the Senior Occupational Therapist/Physiotherapist role at Lancashire Care NHS Foundation Trust. Overview An exciting opportunity to join Lancashire South Cumbria Foundation Trust's Central and West Network as a Senior Therapist. The role sits within acute therapy delivery within Care Connexions as part of the Urgent Community Response, Virtual Ward and Home First teams, providing therapy assessments in patients' home environments to support admission avoidance and early discharge from hospital. Responsibilities Undertake clinical triage to support timely discharges and admission avoidance within the Care Connexions single point of access; may cover across the wider community network. Work as part of a multidisciplinary team to ensure appropriate intermediate care services are commissioned based on patient needs. Promote continuity of care from the acute setting into the community; support home-based recovery after assessment. Assess patients' function and care needs in their usual place of residence; contribute to discharge-to-assess models. Requirements / Qualifications HCPC Registration Degree or Diploma in Occupational Therapy/Physiotherapy Evidence of continuing professional development in relevant areas Experience and ability to work effectively within a MDT Evidence of clinical topic training / staff presentations Evidence of supervision of assistant staff / students Treatment of a broad range of conditions; demonstrable experience in leading and implementing clinical change Details Band 6 Salary £38,682 to £46,580 per annum pro rata Contract: Permanent Working pattern: Part-time Location: Leyland House, Leyland, PR26 6TY About Us LSCFT values diversity and inclusion and welcomes applications from all ages, backgrounds and identities.
