OverviewWe are recruiting a Supported Living Coordinator for our supported living and outreach service in Blackpool, which provides person centred support to adults with learning disabilities, some of whom have autism spectrum conditions and complex needs. We are looking for an experienced practitioner with management experience to provide care and support to people with learning disabilities.ResponsibilitiesDeveloping the independent living skills and community inclusion of service usersIdentifying and accessing activities with service users to promote valued and meaningful lifestylesProviding respectful personal care, emotional and practical support to service usersCollaborating with service users, families, and the multi-disciplinary team to maximise service user outcomesImplementing Creative Support principles, policies, procedures and guidelines and ensuring that the service fulfils CQC requirementsSupervising and supporting staff to best meet service user needs and preferencesSupporting service users with behaviour that challenges services and implementing positive behaviour supportRequirementsExperience is essential for this roleAccess to a carYou will be expected to work flexibly to meet the needs of the service, including evenings and weekendsBenefitsProbationary bonusPension contributions28 days Annual LeaveCompany paid enhanced DBSAbout usWe are a passionate, inclusive, and anti-racist organization - Stonewall Diversity Champion, Disability Confident Employer who have recently received Investors in People Gold awarded.Application and eligibilityVacancy Reference Number: 87497Applications for this role must be submitted via the Creative Support website using the above vacancy reference number.
