Are you passionate about providing a patient centred support service and want to deliver excellent and compassionate care to people over the age of 18 that live in Bedfordshire?We are looking for a dynamic, motivated and enthusiastic person who has excellent communication, IT literate and problem solving skills alongside a passion for patient centred care.Main duties of the jobYou will be part of a MDT team in which you will receive clinical and managerial support, which is also part of a wider MDT team that is aligned to social care professionals in reflection of the NHS 10 year plan.You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, you will be following a care plan devised by the community nursing staff who you will be working alongside. Access to a car in order to carry out service duties is essential.A full induction programme will be provided.Location: South Bedfordshire (Dunstable and Leighton Buzzard)We have posts available to work alongside the Dunstable community nursing team delivering care to people living in Dunstable, Houghton Regis, Caddington, Toddington and surrounding villages and the Leighton Buzzard Community nursing team, delivering care to people living in Leighton Buzzard, Linslade, Hockliffe and the surrounding villages.The Community Nursing Service operates between 08.00 to 21.00 hours and the working pattern will include a variety of rotational shifts which will include weekend, evening and Bank Holiday working.We have the following posts available:2 x Full time positions (37.5 hours per week)2 x Part time positions (1 x 18.75 hours per week and 1 x 15 hours per week)Person SpecificationEducation/QualificationNVQ level 2 and / or significant care experience and willing to work towards NVQ Level 3Completed a recent Care CertificateExperienceWorking in a Community Health or Social Care settingKnowledgeGood communication skills both verbal and writtenRehabilitation SkillsBasic ability to use Microsoft packages - Word, Excel - and databasesAble to support clients, carers and family during times of emotional distressAble to cope with the changing demands and unpredictable nature of the service.Be able to undertake frequent standing, kneeling, crouching, hoisting and other moving and handling proceduresRegularly move and handle clients that may require moderate physical effort, in confined spaced and often unsuitable environment, using relevant practical experience, equipment and skills and enable optimum safety and best practiceOtherAble to demonstrate personal and professional integrityWilling to work to meet the needs of the service (to include evenings, weekends, bank holidays)Be a team player and support colleagues as requiredBe able to drive around Bedfordshire in own car to travel between clients' homes and other venuesHave a full Drivers licenceHave a vehicle to use for work purposesAble to adapt to changeDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
