They will have the understanding and ability to deliver delegated care under the direction of their supervisor, and support the multidisciplinary team in the delivery of high quality care to enable individuals to achieve their person-centred goals in their own home.The post holder will carry out well-defined routine duties in accordance with the individual's care plan, promote independence and check on the individual's overall progress, comfort and wellbeing.When working as a lone worker the post holder will act on own initiative to make a situation safe and report any concerns as required.About usKingswinford & Wordsley Primary Care Network is a network of 5practices with a list size of over 50,000 patients. This role exists tosupport the delivery of the Kingswinford & Wordsley PrimaryCare Network agenda. The post will form an integral part of the PCN leadershipteam working alongside the Practice Managers and the Clinical Directorand the PCNPartners.Job responsibilitiesSupports health promotion and prevention activities across the PCN, including smoking cessation, weight management, and lifestyle interventions.Plays a key role in vaccination and immunisation programmes within the PCN.Works within a Multi-Disciplinary Team to support patients with long-term conditions and health inequalities.Provides clinical support to registered nurses and other healthcare professionals within the PCN.Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives.QualificationsMaths QualificationVenepunctureNEWS2 scoringExperienceExperience of working in a General Practice.Use of IT systems including EMIS, Care Notes, Microsoft office, email and database systems.Preparing and maintaining environments and equipment before, during and after patient care.Sterilising, cleansing and maintenance of surgical equipment.Covid vaccine re-stocking and stock rotation.Work closely with PCN Care Coordinator.Has relevant experience in health promotion, prevention, and supporting long-term condition management;Works under the supervision of a registered clinician while developingcompetencies in community-based care; andIs working at Support Worker Level Practice as described in the PrimaryCare and General Practice Nursing Career and Core Capabilities Framework.Experience using EMIS.RequirementAccess to a carValid UK Driving LicenceDisclosure and Barring Service CheckThis post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
