Overview A member of the Mid Essex Dementia & Frailty Pathway will provide high‑quality, evidence‑based, holistic care to patients of any age with dementia and older adults with functional mental health needs and age‑related frailty. The role involves assessing, planning, implementing and evaluating care packages based on individual service user needs, under the guidance of the Nurse Practitioner. Responsibilities Assess, plan, implement and evaluate packages of care for individual service users. Provide day‑to‑day supervision and management of junior team members within the professional group, in line with Trust HR processes. Collaborate with key partner organisations, service users, carers, clinicians and other practitioners within the multidisciplinary team to deliver a 7‑day a week, 24‑hour service where required. Operate as a community position; use a valid driving licence and access to a car as needed. Maintain required COVID‑19 vaccination records and comply with related regulations. Participate in safer recruitment and safeguarding processes. Candidate profile The successful candidate will be trustworthy, positive about change, independent and autonomously driven, and will possess excellent verbal and written communication skills, strong interpersonal skills, and a passion for delivering the highest level of patient care and service delivery improvements. Qualifications Understanding and ability to conduct assessments, reviews of care plans and risk assessments. Experience or knowledge of the role of a care coordinator and ability to manage a caseload. Knowledge of health legislation, CPA, Care Management, Mental Capacity Act, Mental Health Act 1983, safeguarding, NHS Constitution, and risk assessment/management. Experience in performance management and supervision of lower‑banded staff, and ability to deliver teaching and learning to them. Excellent IT skills (Word, Excel, PowerPoint, Outlook). Flexible approach to work and willingness to travel across sites and Trust boundaries. Valid driving licence.
