Diabetes Practice Nurse

Job Summary We are seeking to recruit an experienced Diabetic Practice Nurse at our highly successful 14,000 patient practice in Dorking. Hours for this post: 25 to 37.5 hours a week. We are a friendly practice with a large supportive nursing team. Main Duties To lead and deliver a high-quality, evidence-based diabetes service for the practice population. The postholder will manage a defined caseload of adults living with Type 1 and Type 2 diabetes, providing assessment, treatment, education, and ongoing review to optimise care, prevent complications, and support self‑management. They will work collaboratively with GPs, practice nurses, healthcare assistants, and allied health professionals to ensure safe, consistent, and coordinated care across the patient journey. The postholder will also act as a clinical resource within the practice, contributing to the professional development of colleagues and the continuous improvement of diabetes care. Clinical Patient Care & Management Maintain a defined caseload of patients with diabetes, delivering ongoing assessment, monitoring, review, and follow‑up. Undertake complex assessments, develop individualized care plans, monitor investigations (HbA1c, lipids, renal function, etc.). Initiate, titrate, adjust and monitor diabetes medications, including insulin therapies. Educate patients in self‑management, provide structured education programmes (DESMOND/DAFNE), conduct group and one‑to‑one sessions. Identify and refer patients with complications; provide telephone, virtual, or in‑person follow‑up; undertake home visits when necessary. Ensure continuity of care and coordinate with secondary care and community services. 2. Clinical Leadership & Service Development Lead and coordinate diabetes clinics, scheduling, patient recall and quality assurance. Develop, implement, and review local pathways, policies, and protocols. Audit and monitor diabetes care metrics, drive improvement initiatives. Act as a specialist resource and mentor for practice staff. Deliver or facilitate in‑house training and case‑based discussions. Contribute to health promotion and prevention programmes. Liaise closely with local diabetes networks and secondary care providers. Support implementation of new technologies such as CGM and digital tools. Lead or contribute to quality improvement projects and service redesign. 3. Professional and Quality Assurance Duties Maintain up‑to‑date knowledge of national and local diabetes guidance. Maintain accurate clinical records in accordance with NMC and practice standards. Participate in clinical governance processes, risk management, incident review, medicines safety. Undertake regular audits, service evaluation, research, outcome measurement. Engage in continuing professional development (CPD) and reflective practice. Maintain professional registration and, if applicable, independent prescribing qualification. Adhere to the NMC Code, confidentiality, data protection and safeguarding. Administrative and Professional Responsibilities Accurately record all consultations, care plans and interventions on the clinical system. Ensure appropriate coding to support QOF and enhanced service reporting. Maintain and review diabetes registers. Contribute to data collection, audits, and reporting. Participate in policy review and development. Attend clinical meetings, case reviews, and multidisciplinary discussions. Maintain active NMC registration, attend training, study days and supervision. Maintain confidential, safeguarding, equality and diversity commitments. Participate in CPD and annual performance reviews. Person Specification Experience (Essential) Experience with diabetes and other chronic disease management. Experience working autonomously. Experience of infection prevention and control measures. Clinical Knowledge and Skills (Essential) Work within own scope of practice and know when to refer to GPs. Strong clinical system IT knowledge and accurate clinical note recording. Communicate complex and sensitive information effectively. Understand safeguarding adults and children; knowledge of GP surgeries, QOF, enhanced service. Communicate and understand patient needs. Qualifications (Essential) Registered General Nurse. Qualification in Diabetes. Evidence or experience at relevant level of practice. Desirable Broad knowledge of clinical governance. Awareness of public health issues locally. Knowledge of national standards (NICE guidelines). Mentor or teaching qualification. Non‑Medical Prescriber. Robust communicational skills and sensitivity.
