The successful candidate willbe working as part of a multi-disciplinary team including Nurse Practitioners,Community Paramedics, Nursing Associates, Clinical Pharmacists and SocialPrescribers delivering care within their scope of practice to our patientpopulation both in the surgery and on home visits.Main duties of the jobTheAdvanced Nurse Practitioner(ANP) has been assessed as competent inpractice using their expert clinical knowledge and skills. They have thefreedom and authority to act and make autonomous decisions in the assessment,diagnosis and treatment of patients.TheANP provides an opportunity for patients to avoid the need to be referred to aGP, thereby enabling them to receive timely care and negate unnecessary delayin receiving treatment.ANPsin primary care can develop close, long-term relationships with their patientsand work in partnership with them to achieve optimum health. They areautonomous in making decisions based on assessment, diagnosis andinterpretation of test results. They can independently prescribe appropriatemedication, evaluate or refer to other specialists if necessary.About usMarlow Medical Group offers a supportive environment where you will autonomously assess, diagnose, treat, and evaluate a wide variety of patients and pathologies in line with local and national guidelines.The post holder is an experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking right through to diagnosis, treatment, and evaluation of care.The post holder will be under the supervision of a senior clinician, providing a range of services, such as triage, telephone and face-to-face appointments, clinical decision-making, referring patients appropriately and interpreting results.Asking advice or learning is very much encouraged and career development is supported.Job responsibilitiesKey responsibilities:Work collaboratively as part of a multidisciplinary team to deliver a comprehensive spectrum of clinical services, encompassing triage, assessment, diagnosis, treatment, and the management of acute and chronic conditions for patients of all age groups.Conducting assessments and formulating management plans in close collaboration with patients and other healthcare team members.Demonstrate critical thinking in the clinical decision-making process, referring or signposting patients who present with undifferentiated or undiagnosed conditionsrelating to minor illness or minor injury.As an independent prescriber: prescribe/ de-prescribe safe, effective, and appropriate medication as defined by current legislative framework.Request and interpret investigations such as blood results and cultures.Maintaining meticulous and up-to-date medical records, aligning with clinical and professional standards.Embrace opportunities to work and liaise with others, both internally and externally, across multi-professional and multi-agency boundaries, in the interests of improving patient outcomes.Provide assessment, screening treatment, services and health education advice.Assist with the organisation and co-ordination of the provision of nursing services for the practice.Provide nursing treatment to patients in participation with general practitioners, using agreed protocols, including the taking of cervical smears.Provide general and specific health screening to the practice patients (within the agreed protocols) with referral to general practitioners as necessary.Be competent in administration of childhood immunisation and travel vaccination.Be competent and confident in managing people with long-term health conditionsAssess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosisProactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).Diagnose and manage both acute and chronic conditions, integrating both drug-and nondrug-based treatment methods into a management plan.Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.Work with patients in order to support compliance with, and adherence to, prescribed treatments.Provide information and advice on prescribed or over-the-counter medication, medication regimes, side-effects and interactionsSupport patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.Implement and participate in vaccination and immunisation programmes for both adults and children.Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatmentRecognise and work within own competence and professional code of conduct as regulated by the NMCProduce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.Prioritise, organise and manage own workload in a manner that maintains and promotes quality.Deliver care according to NSF, NICE guidelines and evidence-based careMaintain and control injectable and oral drug stocks in the treatment room and doctors surgeries.Advise manager regarding health and safety aspects.
