Job Description The successful candidate will hold a Nursing Associate Foundation Degree, and work as an Associate Practitioner in a highly active community team, contributing to the provision of 24/7 specialist palliative/end‑of‑life care. Responsibilities include providing care, advice and support to patients, families and other healthcare professionals within the community. The post holder will deliver palliative and end‑of‑life care on the Mountbatten multidisciplinary community services team, providing nursing interventions and ongoing assessment under the direct clinical leadership of the Clinical Nurse Specialists/Senior Nurse on Duty, reporting to the Community Team Leader. Key Tasks Develop and maintain communications with patients, carers, other professionals and volunteers. Work closely with members of the multidisciplinary team and outside agencies to ensure the provision of individualized care. Demonstrate awareness of limits of practice and seek support and guidance as appropriate. Escalate any concerns as appropriate. Support with telephone advice for patients, carers and other professionals, as required from the Community Coordination centre. Promote a safe environment for all patients, carers, visitors and staff in the patient’s preferred place of care. Person Specification Skills Strong communication skills to explain complex and sensitive information effectively in both written and oral form to patients, families, carers and other professionals. Good IT skills in Microsoft and email applications. Knowledge of specialist symptom management. Experience Experience of dealing with vulnerable adults in a care setting. Experience of working collaboratively within multidisciplinary teams and the ability to form good working relationships at all levels, including volunteers. Experience of working in a person‑centred/personal outcomes approach. Abilities Physical capability to perform the role and carry out responsibilities required by the duties of this post. Ability to be an effective advocate for the patient with multidisciplinary teams across all healthcare settings. Capability to work shift patterns on a rota basis, including nights, weekends and bank holidays. Other Full driving licence and use of vehicle for work purposes with business insurance. A willingness to introduce change for the benefit of patient care. Qualifications Registered Nursing Associate on the NMC Register / Nursing Associate Foundation Degree qualification. Care Certificate qualification / NVQ Level 3 in Health and Social Care.
