Role Overview To deliver high quality person-centred support to customers living in supportedaccommodation service in accordance with the Person Centred principles, enabling clients to sustain their tenancies by promoting independence, freedomand choice. Key Responsibilities: Assess referrals and engage with prospective clients using a person-centeredapproach. Key to this role will be liaising with health professionals, other agenciesand colleagues as appropriate. Ensure that prospective and current clients are fully aware of the supportavailable, their financial obligations, rights and responsibilities under theirtenancy/occupation arrangements and all related procedures. Agree and review client Independent Living Plans and Risk Assessments on a regular basis, deliver contracted hours of support to achieve the requiredoutcomes. Flexibly deliver the support identified in an Independent Living Plan throughregular person-centered contact as appropriate, taking follow up action andcoordinating wraparound support if required from other agencies (for exampleAdult Social Care, Criminal Justice, Health services) and internal departments. Forge and maintain links with partner agencies and other key stakeholders asappropriate to ensure that good communication and co-operation is maintained. Report any concerns relating to safeguarding of vulnerable people using thecorrect procedures, attending case conferences and other meetings as required. Ensure that all relevant documentation and records are completed and submittedin an accurate and timely manner. Actively contribute to the collection of data to be used in statutory returns. Handle complaints and representations in accordance with A2Dominion’s policyand undertake any necessary investigations. More about you: A working knowledge and understanding of how to support people in achievingand maintaining independence Housing or Support related qualification desirable but not essential Experience of providing support to diverse client groups Experience of working with clients experiencing substance misuse and mentalhealth. Educated to GCSE level or equivalent in Maths and English (Grades A*- C) Ability to work under own initiative Ability to network and build strong relationships with partner agencies,community and key stakeholders. Organisational skills, including time management and ability to meet targets Ability to use IT packages and management information systems. Ability to deal with distressed and aggressive clients.
