This opportunity is ideal for a newly qualified nurse looking to begin their career in a supportive environment, or for an experienced practitioner seeking to further develop their skills in community-based learning disabilities nursing. You will be joining a well-established and experienced team of Learning Disabilities Nurses who are committed to delivering high-quality, person-centred care. Comprehensive support and mentorship will be provided, along with access to funding for continuing professional development to support your growth and career progression. Following a structured induction period, you will manage a caseload of individuals with a range of healthcare needs. Your role will involve working directly with service users, as well as collaborating with families, carers, and social care professionals to ensure holistic and effective care planning. Main duties of the job To provide specialist support, advice and guidance to individuals, families and carers. To provide specialist support as part of the multi‑disciplinary team. To complete specialist assessments as clinically indicated. To participate in PBS training and offer specialist support within the PBS framework. To participate in referral meetings, team meetings and wider MDT meetings. To contribute to team and service development. To carry an individual caseload. To keep up‑to‑date with research and development in your fields of practice. To attend training relevant to the role. To maintain NMC registration and remain up‑to‑date with research and development in learning disability nursing practice. To complete individual risk assessments under supervision and to put a safety plan in place if needed. To communicate with social workers, care providers and other agencies and services. To supervise and assess student nurses on placement with the team.
