Overview Working in a small residential home for adults with acquired brain injuries in Burntwood, Staffs. Part of a team providing direct personal care and rehabilitation to clients in the home. Receive ongoing training, guidance and support. Your work will require you to champion and actively maintain the systematic approach to care and rehabilitation adopted by the organisation. This will require collaborative working, monitoring of client’s goal plans, and a focus on progress and positive outcomes. You will construct client goals through continuous assessment using the assessment documentation and by liaising with the Quality and Rehabilitation Systems Manager (QRSM), Home Manager, Deputy Home Manager, and clients. You will organise your time effectively to carry out role-specific activities supporting the systematic approach to care and rehabilitation. You must be able to drive and maintain a current UK driving licence. You will work with other professional disciplines, conduct research, organise meetings and carry out assessments. At all times you must maintain a professional outlook and act as an ambassador for the organisation. Regular training and education will be required, and you should actively improve your knowledge base/skills independently and attend compulsory monthly support meetings with the Home Manager to address performance and other work-related issues. Responsibilities Work closely with the Quality and Rehabilitation Systems Manager, Home Manager, Deputy Home Manager, care support staff who are named carers for individual clients and clients to devise care and rehabilitation plans. Complete and keep up-to-date client assessments for all clients in the Home. Routinely liaise with the Quality and Rehabilitation Systems Manager in the development of client assessments and care and rehabilitation goal plans. Continually evaluate and progress client goal plans. Write a report based on up-to-date assessments in time for each client review. Manage your time effectively to maintain your role. Work directly with clients in the implementation and monitoring of goal plans. Undertake direct care activities including personal care as part of the staff team, while maintaining time management. Ensure that the Home Manager and Deputy Home Manager are fully informed and kept up to date with care and rehabilitation plans and the rationale behind them. Collaborate with clients and their named care support staff in prioritising and developing care and rehabilitation plans. Liaise with other health and social care professionals as required in the performance of your role as Rehabilitation Coordinator. Organise client reviews, complete the agenda and invite pertinent others. Complete the IPP for each client following each review in a timely way, ensuring IPP outcomes progress as the IPP period progresses. Research the environment to identify suitable resources to support clients’ care and rehabilitation plans. Maintain and further develop knowledge and skills pertinent to the role through active research. Maintain a safe and healthy environment for all clients and work to establish a comfortable, homely environment. Work within the statutory regulations and conditions relating to Residential Homes. Maintain relevant certificates (first-aid, food hygiene and manual handling). Maintain an up-to-date knowledge of safeguarding vulnerable adults. Administer medication to clients in line with medication training directives and home policy and procedure. Respect the rights, privacy and dignity of all clients. Build a network of resources for clients. Drive and hold a full, clean, current UK driving licence and be able to drive the vehicle(s) attached to the home. Actively maintain awareness of current policies and procedures and adhere to them. Undertake training/education at the discretion of the Employer. Use a computer to record information and undertake searches. Attend compulsory monthly support meetings with the Home Manager to address performance and other work-related issues. Maintain awareness of the management and operational systems being implemented in the home and demonstrate understanding of them. #J-18808-Ljbffr
