Overview Job Summary: An opportunity has arisen for enthusiastic individuals to join our inpatient specialist dementia ward based in Exeter. Belvedere Ward provides high quality of care and treatment for people who have complex health needs and a cognitive impairment. We provide a multi disciplinary assessment of patient need to inform their current needs and future care needs. The team are an enthusiastic and highly motivated team, with a passion for working with older adults. We work closely as a multi-disciplinary team and work well together allowing us to better support our patients and one another. We aim to support the overall health and wellbeing of our staff, keeping them physically and psychologically well which is important in these current times. Job Purpose & Duties To assess care needs, develop, implement and evaluate individual care plans. To provide named nurse role to people who use our service. To work alongside families and carers to provide person centred care and to meet the needs of our patients and others that are important to them. To participate in the internal rotation system including weekend working. To be a proactive, skilled member of the ward Multi-Disciplinary team. To demonstrate effective leadership and assist with providing advice and guidance to non qualified staff and students. To act as shift co-ordinator and, in the absence of the Manager as required, act up on the ward. To supervise others in line with Trust policy. To act as named nurse for a cohort of patients and to assess complex health and wellbeing needs, update records on electronic clinical records, and monitor/review care plans. To provide robust risk assessments and ensure they are updated regularly on electronic clinical records. To maintain professionalism at all times and practice in a non-judgemental manner; to ensure safe practice in line with NMC, government and Trust policies/procedures; maintain accurate documentation on the correct recording systems. Communication & Working Relationships To have excellent written and verbal communication skills. To be proficient in the use of electronic systems. To maintain professional working relationships with other members of the Multi-disciplinary Team. To liaise with carers, families and user groups and representatives, and with professionals and agencies allied to the holistic care of the person using this service (e.g., GP, Social Worker, Housing). Planning & Organisation To act as shift co-ordinator. Act up (take responsibility for the ward) in the absence of Manager as required. Supervise others in line with Trust policy. Policy, Service, Research & Development To be aware of national and professional developments. To participate in ongoing training and education progress. To demonstrate evidence based practice and participate in research/audit/pilot projects. To be aware of current initiatives in inpatient care. To promote service changes with positive professional attitude and fully engage in service changes. Finance, Equipment & Other Resources To ensure Trust policies and procedures are followed in relation to the handling of monies, property etc. belonging to people using the service. To demonstrate effective risk management skills. Use Trust resources in a sustainable, cost effective and appropriate way. Supervision, Leadership & Management To demonstrate and develop effective leadership/management skills. To provide Supervision and Appraisals to appropriate/allocated staff. To participate in the role of Mentor to learners and Preceptor to newly qualified staff where appropriate. To participate in audits and support learners regularly, including pursuing profession-specific training to the required standard. Information Resources & Administrative Duties To ensure accurate documentation and working knowledge of patient record systems. To engage with and participate in the use of electronic recording systems and training when required. To contribute to reports for other professionals (e.g., panel for continuing care). Other Tasks To demonstrate a working knowledge of NMC codes of conduct and comply with them. To demonstrate a working knowledge of government health and social care policy. To have a working knowledge of PREP requirements and maintain personal, professional development. Report any incident of unprofessional behaviour, accident/incident, untoward occurrence, or faulty equipment immediately to Ward Manager/Team Leader. To take an active role in own supervision and continuous professional development. Knowledge, Qualifications & Skills Knowledge - Essential: Knowledge of ordering, storing and administering medications; knowledge of possible side effects; knowledge of the Mental Health Act 1987 (including updates); knowledge of the Mental Capacity Act & Deprivation of Liberty safeguards. Qualifications - Essential: RMN/Registered Nurse (Mental Health) with current NMC registration; willingness to complete Trust Mandatory Courses and relevant Physical Interventions training as required. Skills & Abilities - Essential: Excellent verbal and written skills; ability to form positive therapeutic relationships; excellent IT skills; ability to lead MDT meetings; risk assessment; maintain personal boundaries; work within data protection and confidentiality guidelines; able to work as part of a team and within internal ward shift rotation including night duties; ability to plan and implement all aspects of nursing skills; ability to demonstrate leadership and act as role model. Experience Essential: Experience of Adult and/or Older Persons Mental Health Nursing and the nursing process. Desirable: Previous experience of working with people with dementia.
