An excitingopportunity has arisen for a dynamic staff nurse to join the bank workforcewithin First Community Health and Care working in the Responsive Services Team.Responsive Servicesconsists of the Urgent Community Response Team, Intermediate Care Team, VirtualWard and Referral Hub.This post holder will work in collaboration with Primary Care andSecondary Care Colleagues, Urgent Community Response, Intermediate Care(Responsive Services), Community Nursing and other Specialist Health Care Teamsacross First Community, statutory and voluntary agencies to deliver excellentstandards of patient care. Assessing and treating patients who are acutelyunwell and requiring support to avoid admission to the acute hospital or thosewho have recently been discharged home. Providing practical care, education andtraining for patients and carers admitted to responsive services.Main duties of the jobYou will complete assessments in the patients home and undertake treatments including IV medications, wound assessment, and management. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own home.The candidate will be able to adapt to change and work in a busy environment. Excellent communication skills are essential to enable liaison with all our health and social care partners in the management and assessment in the community.Due to the nature of the role it is essential that applicants are able to drive, have a car suitable for business purposes and have a clean driving licence.FCH&C is committed to safeguarding and promoting the welfare of vulnerable adults and children and expect all staff to share this commitment. FCH&C will support personal development through the annual appraisal process and in house training.The service runs from 8am-8pm 7 days a week, 365 days a year, the post holder will be expected to work on a rota basis.
