Job DetailsPosting Date: 12 June 2024Salary: Not specified (Negotiable)Hours: Full timeClosing Date: 04 July 2024Location: Market Rasen, LN8 3BPCompany: NHS JobsJob Type: PermanentJob Reference: A4457-24-0004SummaryPrimary Care Network Clinical PharmacistThe Primary Care Network (PCN) Clinical Pharmacist will support East Lindsey GP Practices by implementing effective medicine management strategies, identifying areas for improvement, and driving change as part of the NHS England Clinical Pharmacists in General Practice pilot.Patient ServicesManage repeat prescriptions, synchronize medication, and review discharge and clinic letters.Address urgent script requests and conduct face-to-face reviews for complex medication regimes.Collaborate with hospital pharmacists and conduct annual medication reviews for patients with long-term conditions.Conduct medication reviews and de-prescribing in nursing, care, and residential homes.Provide consultations in clinics and as requested by doctors, both in practice and domiciliary settings.AdministrationProvide regular support and feedback to practices on prescribing action plans.Maintain accurate patient medication records and update the clinical team on prescribing policies.Complete Clinical Commissioning Group (CCG) actions and disseminate prescribing tips to the team.CommunicationCommunicate effectively with team members, patients, and carers.Recognize and respond to alternative communication needs.Report to Practice Managers, GP Partners, and the Senior Clinical Pharmacist Supervisor.ConfidentialityHandle patient and business information with the utmost confidentiality.Adhere to practice policies regarding the protection of personal and sensitive data.Health & SafetyPromote and manage health, safety, and security according to practice policies.Ensure compliance with infection control procedures and maintain a clean and safe working environment.Undertake regular health and safety training and manage the training of others.Equality and DiversitySupport the rights of patients, carers, and colleagues by respecting privacy, dignity, and beliefs.Behave in a welcoming, non-judgmental manner that respects individual circumstances and rights.Personal/Professional DevelopmentParticipate in ongoing training, including independent prescribing if not already qualified.Engage in annual performance reviews and take responsibility for personal and professional development.Demonstrate skills and activities to peers and contribute to team training.QualityMaintain high standards within the practices and contribute to the continuous improvement of team performance.Work effectively with other agencies to meet patient needs and manage time and resources efficiently.Contribution to Service ImplementationApply practice policies, standards, and guidelines to daily work.Engage in discussions about the impact of policies on work and participate in audits as appropriate.Apply for This JobFor more information or to apply for this position, please visit the NHS Jobs website and refer to job reference A4457-24-0004. #J-18808-Ljbffr
