Service Overview The service aims to: Ensure the delivery of least restrictive and most cost-effective support models at a scheme level, enabling individuals to achieve their outcomes. Review the quality and affordability of accommodation. Ensure the delivery of quality care and support at a scheme level. Essential Experience You will need to have experience of: Performing reviews to ensure that assessments and support plans accurately represent the support outlined by the service and meet the individuals' outcomes. Translating review information into structured documentation to enable the calculation of proportionate support. There is plenty of hybrid working available. Please get in touch for more information.
