Community Falls Nurse

To support the development and delivery of an enhanced community falls prevention and management service across South Tyneside. The role will focus on implementing a new falls pathway aligned with national guidance, providing clinical expertise, education, and direct patient assessment to reduce falls incidence and improve patient outcomes. Main duties of the job Clinical Assessment and Management Conduct comprehensive falls risk assessments for patients with recurrent or complex falls. Deliver nursing-led falls clinics in collaboration with the physiotherapy and frailty teams. Develop individualised care plans and coordinate referrals for further intervention where necessary. Support early intervention for frail patients to reduce the likelihood of repeat falls. Education and Training Deliver training and educational sessions for health and social care staff on fall prevention, risk identification, and environmental safety. Contribute to the creation of digital and written learning resources for staff across primary and community care. Support partner agencies in public education and falls awareness activities. Pathway Development and Quality Improvement Assist in implementing a standardised falls assessment tool for community and care home settings. Work collaboratively with the Community Frailty Team to ensure seamless transitions of care. Collect and analyse data to monitor pathway effectiveness and outcomes. Collaboration and Communication Liaise with multidisciplinary teams (MDT), including GPs, physiotherapists, occupational therapists, and social care professionals. Provide expert advice on falls prevention strategies across community settings. About us STHC is a not-for-profit GP Federation, set up in 2017, to bring together GP practices to work collaboratively, delivering services at scale across the borough of South Tyneside and is formed from 18 of the 21 practices across South Tyneside. Qualifications Registered Nurse (Adult) with current NMC registration. Teaching qualification Experience in community nursing, gerontology, or falls prevention. Strong understanding of frailty management and risk assessment. Excellent communication and teaching skills. Ability to work autonomously and within a multidisciplinary team. Clinical knowledge and skills appropriate to falls prevention Understand the importance of evidence-based practice Broad knowledge of clinical governance Ability to record accurate clinical notes Ability to work within own scope of practice Polite and confident, flexible and cooperative Motivated, forward thinker Problem solver with the ability to process information accurately and effectively, interpreting data as required High levels of integrity and loyalty Sensitive and empathetic in distressing situations as well as ability to work under pressure/in stressful situations Enhanced Disclosure Barring Service (DBS) check Occupational health clearance Meet the requirements and produce evidence for revalidation Evidence of continuing professional development (CPD) Access to own transport and ability to travel across locality on a regular basis Experience of working in a primary care environment Knowledge of public health issues in the local area Awareness of issues within the wider health arena Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. #J-18808-Ljbffr
