Social Worker – London Borough of Hillingdon The London Borough of Hillingdon is looking for an experienced Social Worker to work in the Mental Health team, supporting residents in the community and helping them achieve recovery and independence. Key Responsibilities Complete Care Act assessments and Mental Capacity Act assessments. Make placement requests and manage the Reablement / Discharge to Assess (D2A) process. Handle complex cases, including housing referrals and homelessness applications. Work in partnership with health professionals such as hospitals and CCGs. Attend and contribute to multi‑disciplinary team meetings, family meetings and best‑interest meetings. Carry out Section 42 safeguarding investigations. Provide supervision to both newly qualified and unqualified practitioners within the team. Undertake Practice Educator training where relevant. About You You should demonstrate strong values, high professional standards and a genuine commitment to improving lives. You will be confident in advocating for individuals’ rights, flexible in your approach and collaborative in practice. You will use your social work skills, knowledge and values creatively to engage with individuals and promote recovery and independence.
