AHP Lead & Manager Living Well Service Join to apply for the AHP Lead & Manager Living Well Services role at North London Hospice AHP Lead & Manager - Living Well Service We are looking for an enthusiastic and innovative Allied Health Professional to join our community team to help develop and deliver an array of interventions that support people with life-limiting illnesses to acquire the knowledge and skills that focus on self‑management, enabling them to take ownership of their lives and live a full, meaningful life. Person‑centred services that focus on what’s important to them, their preferences and wishes. You will be the line manager of a small team of staff, leading and participating in individual and group‑based therapy programmes and leading community engagement to understand the needs of the local community better, to build relationships and develop services, including partnership working to address these needs; supported by the Head of Community Palliative Care East. The role is 50% clinical and 50% operational management / community engagement to support programme development. North London Hospice is a registered charity, established in 1984, it provides specialist palliative and end‑of‑life care for patients, and support for their families and carers. We work with other health and social care professionals providing expert advice. As well as our inpatient unit at Woodside Avenue North Finchley, our Community Palliative Care Services cover the boroughs of Barnet, Enfield and Haringey, focusing on living well with a life-limiting illness, educating people to self‑manage, empowering and enabling them to live a fuller, more meaningful life, and have the best of life at the end of life. Our community teams consist of Clinical Nurse Specialists, Social Work professionals delivering Patient and Family Support, Physiotherapists, Occupational Therapists, Psychologists, Complementary Therapists and Community Outreach teams, who individually and jointly provide a range of person‑centred services, group activities, practical advice, and bereavement support.
