We work in partnershipwith community services, mental health teams, social care, pharmacy, hospital, and the voluntary sector to deliver proactive, personalised,coordinated, and integrated health and social care for local residents closerto home. The successful candidate will work closely with ourestablished clinical teams to deliver high-quality patient care in aninnovative and collaborative way. Main duties of the job The post holder will be a qualified clinical pharmacist whoworks within their professional boundaries while supporting and collaboratingwith our general practice teams. Theywill contribute to the repeat prescription process, handle acute prescriptionrequests, and carry out medicines reconciliation during transfers of care. Risk stratification using clinical reports,identify cohorts of patients at risk of harm from medicines. Medicines quality improvement analyse,interpret and present medicines data to highlight issues and risks to supportdecision making. Person Specification Experience GP practice experience is desirable, but not essential Experience of using Emis Web. Experience of using TPP SystmOne. Minimum of 2 years experience as a pharmacist, demonstrated within a practice portfolio. Have experience and an awareness of common acute and long-term conditions that are likely to be seen in general practice. Qualifications Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council Able to work flexibly across practices and within a multidisciplinary team Must have own transport and full drivers license Demonstrate understanding of the pharmacy role in governance and is able to implement this appropriately in the workplace.
