Your responsibilities will include supporting clients in maintaining their accommodation, assisting with daily living tasks such as personal care, attending appointments, and ensuring access to appropriate health and social care services. You will also provide tenancy‑related support and work in close partnership with external professionals, families, and community services to promote overall wellbeing and independence. Responsibilities Prepare person‑centred support plans with clients which reflect their views and aspirations and respect their beliefs and opinions, reviewing and amending them as necessary and considering the client's changing needs and wishes. Support clients to fulfil their aspirations and live as independently as possible in accordance with their skills and abilities, including accompanying them on social visits, events, and activities, promoting choice and developing their skills and knowledge. Communicate in an open and transparent way with clients and other team members, acting with integrity and doing what you say that you will do. Provide personal care respecting clients’ choices and dignity, including assistance with washing and toileting where required. Keep accurate records, including daily notes, with full involvement of the client. Ensure all support and care protect clients from abuse, maintain dignity, and support positive risk‑taking. Support quality inspection processes, cooperating with on‑site inspectors in relation to information requests. Work in accordance with Health and Safety requirements such as Manual Handling, Food Hygiene, Fire Safety, infection control, and Risk Assessment procedures. Carry out household duties: cooking, cleaning, laundry, and other domestic duties as required. Provide appropriate level of care and support in accordance with the care plan, including personal care. Qualifications & Skills Understanding of the importance of maintaining accurate written and numerical records. Ability to undertake a wide range of personal care and domestic duties. Ability to carry out the physical requirements of the role including but not limited to climbing stairs, using hoists, pushing wheelchairs, moving people, night work. Willing to learn how to use computer packages such as Word, Outlook and the Internet; willingness to undertake relevant training and qualification in care if required. Ability to work cooperatively as a team member. Effective liaison and communication with carers, families, inspectors and other external stakeholders. Will be compassionate, flexible, proactive with strong commitment to empowering individuals. Adjustments will be considered in line with the Equality Act where these criteria cannot be met due to a disability.
