Job Summary We are looking to recruit a full‑time Associate Transformation Manager on a fixed‑term contract until 31st March 2027. The Confederation Hillingdon CIC is a collaborative organisation that works with general practices across Hillingdon to deliver Better Health through Collaborative Primary Care. We support general practices and other healthcare providers in delivering high‑quality services for patients, improving care by facilitating collaboration, releasing clinical time and developing the primary care workforce. We develop and support GP practices, PCNs and Neighbourhoods, act as the Hillingdon provider‑representative voice, and deliver innovative and transformational services both at scale and complementarily to general practice. Our Values We work together to make a difference for patients We care enough to go the extra mile We support, trust, and empower We sincerely value each other We support Primary Care to own its destiny Job Description – Key Duties and Responsibilities Support Transformation Managers in the development and delivery of plans for Neighbourhoods, PCNs, and practices. Inform the design and implementation of Neighbourhood strategy and development. Work on various HHCP programmes within Neighbourhoods and individual development plans. Assist Transformation Managers with PCN programmes that support Neighbourhood working. Lead programmes such as Enhanced Services and Local Enhanced Services, ensuring SOPs and materials for practices and PCNs are developed. Utilise PCIT and NWL dashboards to help PCNs monitor achievements against contractual targets. Analyse population health and BI data to evaluate neighbourhood development areas and drive continuous improvement. Facilitate and enable transformational change, building excellent functional relationships with multiple stakeholders. Oversee submission of PCN documentation to commissioners in partnership with Clinical Directors and PCN Managers. Identify process improvement opportunities and streamline ways of working. Apply project‑management and change‑management methodologies to deliver key deliverables and KPIs successfully. Support the achievement of Neighbourhood objectives through partnership working and engagement. Investigate best practice, evaluate applicability, and develop implementation plans. Deliver measurable results against outcomes, including clinical improvements and cost savings. Provide and receive complex, sensitive information, presenting to stakeholders in formal settings. Help develop effective networking to share good practice across Hillingdon practices/networks. Coordinate and develop community engagement activities, health education sessions, and local health checks. Produce clear communication promoting PCN services and staff, create engaging content for websites and social media, and manage online engagement metrics. Act as primary contact for VCSE services, maintaining strong relationships for integrated care. Promote a positive image of the Confederation Services externally and internally. Participate in induction programmes, training of new staff, and wider meetings representing the team. Person Specification – Qualification and Experience Essential Qualifications Degree level education or equivalent, or evidence of relevant continuing professional development. PRINCE2 practitioner or equivalent project‑management accreditation. Demonstrated pattern of learning from education, training, and experience. Essential Experience Experience in a project‑management environment covering the full project life cycle. Experience meeting deadlines, targets, and working within budgets. Experience dealing with sensitive and confidential information. Ability to work autonomously and as part of a team. Excellent knowledge of IT systems and software (Outlook, Word, Excel, PowerPoint). Understanding of equal opportunity and diversity issues. Ability to multitask, prioritise, and manage time effectively. Clear communication and presentation skills. Strong coordination and organisational skills. Diplomatic and tactful handling of complex information. Analytical skills for data interpretation. Effective communication pathways with internal and external partners. Problem‑solving and prioritisation skills. Timely reporting and influencing skills. Desirable Experience Experience working with GPs, CCGs, acute clinicians, allied health professionals and broader health and social care stakeholders. Experience managing change to improve quality service delivery.
