By decreasing the workload on general practice teams, they improve overall access to primary care services for patients. Details Date posted: 26 November 2025 Salary: £47,810 to £54,710 a year depending on experience Contract: Permanent Working pattern: Full‑time, Part‑time Reference number: W0062-25-0000‑ENP Job Locations The Health Hub, Andover PCN Chantry Centre Andover, Hampshire, SP10 1RL Job Description – Key Responsibilities As part of the PCN multidisciplinary team you will be responsible for the following: Clinical Delivery: Serve as the Clinical Lead for the Andover Health Hub, providing expert clinical support, oversight, and guidance to the General Practice Assistants (GPAs) and Care Coordinators, ensuring high standards of patient care; act as Clinical Lead during COVID‑19 vaccination campaigns; support the frailty pathway by conducting Structured Medication Reviews, participating in multidisciplinary frailty reviews, and completing home visits where clinically appropriate; provide one clinical session every other week to support the five core network practices. Education & Supervision: Provide clinical supervision, education, and skills development for the Health Hub team, ensuring GPAs and Care Coordinators attain necessary competencies; support workforce development through identifying learning opportunities and fostering continuous professional growth. Administrative Leadership & Support: Act as Infection Control Lead for the Health Hub, ensuring compliance with IPC policies; collaborate with the non‑clinical Health Hub Manager to review and update relevant policies, protocols, and SOPs. Provide one clinical session every other week tailored to practice needs and competencies. Wider Responsibilities Support delivery of structured medication reviews, focusing on polypharmacy and deprescribing. Facilitate and support the development and delivery of innovative training and education to staff. Participate in and/or lead formal training events promoting best practice. Provide professional and clinical leadership and mentorship, acting as a clinical role model. Advise and support on the development and delivery of work streams relating to area of expertise as part of the PCN's strategy. Work proactively with key local and national stakeholders to develop integrated care pathways and holistic models of care. Contribute to delivery of local policy, procedures and protocols for the PCN. Person Specification – Qualifications Relevant nursing or health degree. Registered with the Nursing & Midwifery Council. Post‑graduate qualification at level 7 or above relevant to area of enhanced practice. Clinical supervision training. Independent prescribing qualification.
