Specialty Doctor in Frailty/Intermediate Care A bright opportunity exists as part of the Health Board's Clinical Futures model and redesign work within Frailty/Intermediate Care. The Frailty Service is a multi‑disciplinary service within the Primary Care and Community Division, centred on providing people with care and/or treatment close to home and promoting independence via reablement teams. The aim of the service is to reduce unnecessary hospital admission by providing safe alternate pathways and minimising hospital stay by facilitating early and safe discharge. The redesign of services for older and/or frail people means that you will be joining the Primary Care and Community Services Division at a once‑in‑a‑generation era, helping to develop a ‘best in class’ Frailty/Intermediate Care service model. Main Duties of the Job All posts are based in the community and flexible working plans afford aspirants the opportunity to participate in the management of community inpatients, Same Day Emergency Services, community wards and community Frailty Units. Further training can be developed depending on your specific needs including palliative care, mental health and specialty clinics. The opportunity to expand your skills and knowledge of the community through rotation across the boroughs is also advocated and encouraged. ABUHB is highly flexible in respect of career progression, for example the MRCP(UK). We actively support doctors who may wish to pursue a CESR route for specialist registration. Audit and QI projects are a requirement of the post and we have close links with Cardiff University should you want to be involved in research. Job Responsibilities Support the Frailty/Intermediate Care team on a day‑to‑day operational basis and carry a clinical caseload. Provide a ‘hospital at home’ model of care, 7 days a week, 12 hours a day, as part of the Monmouthshire integrated service. Coordinate and manage patients in the community and in community hospitals under the supervision of the Consultants and/or in close liaison with General Practitioners. Provide medical support for health and social care services that make up the Frailty/Intermediate Care service. Work closely with the Consultants and the Directorate Manager to deliver effective and appropriate intermediate care services to residents of the borough. Play a key role in discharge planning, co‑ordinating approaches with relatives and other agencies. Liaise with patients, relatives/carers and members of the multi‑disciplinary team to ensure patients receive a high‑quality service and are kept informed of progress as appropriate. Maintain contemporaneous record‑keeping, identifying progress and plans in the patient’s case notes. Participate in virtual ward rounds, facilitate Falls multidisciplinary meetings and Consultant ward rounds. Participate in Frailty initiatives, rapid access and co‑geriatric clinics. Actively use the Direct Admission pathway to community hospitals to avoid unnecessary acute admissions to emergency departments. Complete accurate and timely discharge summaries, progress reports and ensure General Practitioners are informed of any impending discharges, changes of medication and death. Assist the Consultants in the development of community, in‑patient (step‑up/step‑down) and outpatient‑based services. Actively participate in multi‑disciplinary audit, clinical effectiveness/health gain initiatives, clinical governance, local clinical audit activities and develop research projects, as appropriate.
