Key Responsibilities: Conduct comprehensive social care needs assessments for young people aged 14–17 to determine Care Act eligibility. Collaborate closely with families and colleagues across various teams, including children’s social work, leaving care, and adult services, to ensure smooth transitions into adult care. Develop, implement, and review personalised support packages, ensuring long‑term sustainability and suitability for each individual’s needs. Apply the Mental Capacity Act and best interest processes, initiating Court of Protection Deprivation of Liberty (CoP DoL) processes when required. Work in partnership with health colleagues in learning disabilities, mental health, and Integrated Care Board teams to deliver cohesive and coordinated care plans. Participate in multi‑agency planning meetings and contribute to transitional safeguarding frameworks. Requirements: Qualified Social Worker registered with Social Work England. Experience working within adult or transition social care services. Strong understanding of the Care Act, Mental Capacity Act, and transition planning. Excellent communication and interpersonal skills, with the ability to engage effectively with young people, families, and multidisciplinary teams.
