Job Overview We’re looking for a Hospice and Palliative Care Nurse Practitioner dedicated to delivering compassionate, patient‑centered care. Responsibilities Provide comprehensive palliative and hospice care to seriously ill patients, aligning treatments with patients’ goals. Collaborate with a multidisciplinary team to deliver optimal care at patients’ homes. Offer ongoing education and support to patients, families, and community. Qualifications Graduate of an accredited nursing program; associate or baccalaureate preferred. At least 2 years of experience within the last 5 years in chronic disease management or long‑term care; hospice background preferred. Reliable transportation; ability to sit, stand, bend, move intermittently and lift 80‑100 lbs.
