This post offers an opportunity to work with the community CHD and Heart Failure nursing service across an identified area in a non‑patient facing clinical admin role supporting the clinical teams. Main duties of the job Be responsible for accepting and completing electronic tasks with the help of the nursing staff as required. Allocate work appropriately using electronic systems. Identify and apply appropriate care plans, setting frequency and dates. Carry out administrative and housekeeping tasks to support the service and patient care. Participate and provide training around the electronic patient management system and functioning of the team in their area. Provide and receive routine information requiring tact or persuasive skills; provide and receive complex or sensitive information; provide advice, instruction or training to the team; deal with and understand behaviours. About us We provide hospital services from two acute sites – Darlington Memorial Hospital and University Hospital of North Durham – and a centre for planned care in Bishop Auckland. We also offer care from community hospitals in Chester‑Le‑Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients’ homes. Full Job Descriptions can be found in the adverts supporting documents. Job responsibilities Electronic referrals and tasks, action appropriately whilst seeking advice if required. Manage own workload with an understanding of what needs to be done, when and why to assist the function of the team. Have the ability to flex and prioritise if required due to an unexpected or urgent need. Provide staff with advice on administrative procedures and help with access as required, including general admin duties around RA agent and IAA functions. Coordinate clinical work for colleagues within SystmOne, reviewing allocated visits to ensure they are geographically close together while maintaining continuity of care. Communicate with patients, relatives and healthcare practitioners effectively; may encounter distressed or challenging individuals at times. Demonstrate effective time management skills and participate in the monitoring and reviewing of team performance. Gather key audit data from electronic systems and produce reports to Clinical Lead/Matron as requested and within timetables. Promote an environment where health promotion and informed patient choice is encouraged and developed to its maximum potential within the available resources. Undertake relevant training to enable provision of a high quality service. Complete effective record keeping in accordance with Trust policy and legal requirements. Commit to continuous personal development/lifelong learning by attending mandatory training, relevant training courses, using eLearning and adhering to all Trust policies and procedures. Attend supervision/1:1 sessions, undertake appraisal preparation, contribute to objective setting and personal development planning. Participate in and contribute to all activities associated with the team’s response to current trends and changes within the NHS. You must be able to produce ALL certificates stated essential in the person specification or you will not be able to complete pre‑employment checks. Person Specification Qualifications NVQ Level 3 or equivalent. Understanding and familiarity with electronic systems such as SystmOne. Experience Experience of using Trust electronic systems such as SystmOne and telephone call handling. Understanding of community nursing caseloads and systems of working. Special Skills & Knowledge Good verbal and written communication skills. Good organisational and facilitation skills. Ability to work independently. Clinical knowledge. Able to assess service user needs and allocate the visit to the appropriate professional to deliver the care.
