Overview As a Staff Nurse you will provide clinical and professional leadership, supervision and support to patients and students within the SMART team. Responsibilities Triage, assess, plan and implement care for patients discharged home under the SMART service in the community. Review and give care to SMART patients attending SDEC, SAU or Hot clinic for review. Administer IV treatments and conduct virtual visits using remote patient‑monitoring pathways. Provide care for OPAT (Outpatient Parenteral Antibiotic Therapy), heart‑failure patients and post‑operative patients. Teach, audit, enforce infection‑control practices and participate in research activities. Coordinate safe, effective and timely care practices with multidisciplinary team members. Actively contribute to innovative initiatives and propose new ideas for service development. Qualifications & Skills Competent cannulation, venepuncture, general surgical wound care and drain care. Ability to work autonomously and effectively within a team. Strong communication skills, sound decision‑making, confidence. Full UK driving licence and access to a vehicle for community visits. Training & Support Specific training will be provided to facilitate the pathways covered by SMART in‑house and in the community. Staff will also receive support and training to safely care for OPAT and post‑operative patients, including cannulation skills. Benefits & Working Practices Flexible working arrangements available.
