If so we want to hear from you About the Service The Day Hospital is an award‑winning service that uses an extended scope of practice framework for holistic healthcare management. The service specialises in management of falls, stroke rehabilitation and Parkinson’s disease, as well as health‑prevention strategies and treats a wide range of multi‑pathologies affecting the older person. We aim to support individuals to maintain independence and manage their long‑term health conditions effectively, reducing hospital admissions and facilitating early supported discharge of patients from hospital with our outreach service, ambulatory care team and collaboration with the acute older persons Hospital at Home service. Key Responsibilities Deliver high‑quality, patient‑centred care to older people with a variety of complex health and social care needs. Work collaboratively within a multidisciplinary team across Christchurch and Poole Hospitals and within the community. Develop skills in holistic assessment, care planning and the delivery of a range of treatments and interventions across different settings. Drive the continual improvement of patient safety, clinical effectiveness and patient experience through implementation of the Trust’s quality strategy.
