Responsibilities Conduct comprehensive assessments and reviews of adult clients to ensure their needs are accurately identified and met. Work collaboratively with multidisciplinary teams to design and implement effective care plans that promote the well-being of vulnerable adults. Offer professional advice and support to clients and their families, helping them access appropriate resources and services. Maintain accurate case records and documentation in accordance with legal, ethical, and organisational standards. Develop tailored support strategies that encourage independence and improve outcomes for adults in the community. Requirements Qualified Social Worker with relevant registration (Social Work England). Experience working with adults in a social care setting, ideally within a review or assessment team.
