What you will do: Assess and provide tailored physiotherapy interventions to individuals within the reablement service. Work collaboratively with an MDT (multi‑disciplinary team) to develop integrated care plans. Support patients in community settings, assisting in their rehabilitation and reablement. Document patient progress and offer insights for case discussions. Provide guidance and advice to patients and their families, enhancing their rehabilitation journey. Requirements of the Physiotherapist: Must possess a valid UK driving licence as a car driver. Registration with the Health and Care Professions Council (HCPC) is essential. Experience working within a multi‑disciplinary team and in rehabilitation is preferable. Seeking a candidate with some post‑graduate experience to manage complex team caseloads effectively.
