Clinical Lead Heart Failure Job Overview We are seeking an exceptional Clinical Lead to drive high-quality, patient‑centred services across our local Camden population. This is a unique opportunity to lead a passionate team, influence service development and ensure people living with heart failure receive the right support, in the right place, at the right time. In this pivotal role, you will champion evidence‑based practice, provide expert clinical leadership, and work closely with primary, secondary, and social care partners to create seamless pathways that empower patients to manage their condition confidently at home. Key Responsibilities Provide evidence‑based treatment in community‑based clinics, patients’ own homes, and secondary care during visits as an independent practitioner. Develop and maintain effective working relationships with all relevant stakeholders to deliver best standards of chronic heart failure care. Advise GPs, medical teams and other health care professionals on medication adjustments, changes in condition or treatment plans. Identify new cases of heart failure and patients on sub‑optimal therapy. Lead and operationally manage the specialist nurses in the Heart Failure team and the clinics, facilitating rotation and cover. Collaborate with services such as GPs, PCN, secondary care to improve patient care and outcomes. Supervise less experienced staff and maintain accurate, legible, comprehensive records. Engage in continuous improvement and quality improvement initiatives. Qualifications & Experience BSc Nursing or equivalent qualification. NMC membership. Working at Masters level in the management of clients with complex respiratory needs (or equivalent). Post graduate clinical courses specific to working with people with heart failure. Non‑medical Prescribing Qualification or willingness to undertake training. Experience working in a health economy similar to the UK. Working at Band 6 or above in a heart failure/cardiology setting. Strong concept of case management and experience of using outcome measures. Evidence of service development & audit experience. Ability to work autonomously in a community setting and manage people with heart failure. Experience with interdisciplinary working and understanding of local services.
