We are launching a brand-new Enhanced Home Treatment Team (Night Function) as part of Phase 1 of the Trust's Crisis Pathway Redesign. This innovative service has been developed in response to the needs of our local communities and aims to provide specialist crisis care overnight, ensuring that people receive timely, compassionate support when they need it most. As this is the first phase of a wider redesign programme, the service will continue to develop and evolve. Staff joining the team will play an important role in shaping how the model grows, helping us to learn, adapt, and embed new ways of working that best meet the needs of our service users. The Enhanced HTT Night Function will: Operate across our 5 boroughs (Wandsworth, Merton, Sutton, Kingston, Richmond) to support adults experiencing mental health crisis during the night hours. Work closely with daytime HTTs, Liaison Psychiatry in Emergency Departments, the Mental Health Crisis Line, and other services to provide seamless, 24/7 support. Offer a truly multidisciplinary approach, bringing together nurses, support workers, and other professionals to deliver safe, effective, and recovery-focused care at home. We are excited to be recruiting Crisis Support Workers (Band 4) to join our Enhanced Home Treatment Team (Night Function). This is a new and innovative service model designed to provide safe, responsive, and compassionate care overnight to people experiencing a mental health crisis in the community. As a Crisis Support Worker, you will work alongside experienced mental health nurses, supporting the delivery of urgent and intensive home-based care at night. You will play a vital role in ensuring that service users receive timely support, practical assistance, and compassionate listening when they need it most. You will: Provide direct care and support to service users in mental health crisis during the night. Support registered practitioners in delivering assessments, interventions, and care planning. Carry out home visits, welfare checks, and practical support alongside qualified staff. Contribute to risk assessments and the monitoring of mental state, wellbeing, and safety. Engage with families and carers to provide reassurance, advice and signposting. Document care clearly and promptly on electronic systems (e.g., RIO). Work closely with colleagues across the Crisis Pathway, Liaison Psychiatry, Emergency Services, and community teams. About our locations: Tooting (Wandsworth) Our Trust headquarters is located within the impressive grounds of Springfield University Hospital in Tooting. This is easily accessible via the Northern Line (Tooting Bec Station) and via private transport. Our main site is located immediately next to Tooting High Street where staff can easily access a range of shops, cafés and restaurants. Clinical Support Assist qualified staff with mental health assessments and interventions, including gathering collateral and supporting engagement Support the delivery of care plans through structured one-to-one support, health promotion, or social support interventions Undertake regular check-in calls, home visits, and community-based appointments alongside or under the direction of qualified clinicians Promote service user safety and wellbeing by helping to implement risk management plans Observe and report changes in service user presentation or risk, escalating promptly to clinical staff Operational Maintain accurate records of interactions with service users and carers on RiO, under the supervision of registered staff Attend zoning meetings, handovers, and team discussions, providing relevant updates Contribute to safe and effective care delivery by assisting with practical arrangements (e.g. booking interpreters, organising transport) Work flexibly across community, clinic, or home settings in accordance with individual care needs Communication & Engagement Build therapeutic relationships with service users and carers, showing compassion, respect, and empathy Support the team in delivering care that is sensitive to cultural, spiritual, and communication needs Provide information to service users and carers on available services, signposting where appropriate Governance & Quality Adhere to all Trust policies and procedures including safeguarding, health and safety, infection control, and information governance Participate in debriefs, supervision, and reflective practice to support learning and wellbeing Contribute to service feedback and improvement activities This advert closes on Wednesday 22 Oct 2025 #J-18808-Ljbffr
